WHAT HAPPENS AT YOUR APPOINTMENT

Before your appointment at the Diabetes Centre, we suggest that patients have their blood samples taken. The blood sample can give both you and your health care professional valuable information about your diabetes. The forms for the blood samples are sent in the post 2 weeks before your appoints are due.

The forms can be taken to the Pathology Department at Airedale Hospital (an appointment is not required) between 9.00 a.m. and 16.00 p.m.or at Skipton General Hospital between 9.00 a.m. – 12.00 hrs). Alternatively you can taken the samples to your family Doctor’s surgery. Please check these arrangements with your family doctor first however.

On the day of your appointment:-
• Bring a urine sample
• Bring your medication or a list of your medication
• Bring a diary documenting your finger prick tests or urine tests

AT YOUR APPOINTMENT

You will see one or more of the following health care professionals, depending on your health needs.
• Doctor
• Nurse
• Specialist Nurse
• Dietitian
• Chiropodist

You may see members of the team before you see the doctor.

At your clinic appointment, you may have your blood pressure checked, your weight measured and vision reviewed. A urine sample may be requested prior to your appointment.

These checks and others are part of vital screening procedures, which are designed to help avoid potential complications of diabetes.

A Specialist Diabetes Nurse is available at each Diabetes Clinic. In between appointments the Specialist Nurses are available for contact or for individual appoints and can be contacted on the above telephone numbers. We cannot guarantee to see those people who call in at the centre without having made a prior appointment.

The need for further appointments at the centre is discussed at the consultation and details are sent in the post when appropriate.

The Centre Team is in the process of developing education sessions for people with newly diagnosed Diabetes. Please discuss your requirements if appropriate with a member of the team.

A regular newsletter is also available through the Centre.

EYE SCREENING

We suggest all people have their eyes checked on a yearly basis. The check is to ensure that no changes occur to the blood vessels at the back of the eye.

If you live in Bradford Metropolitan Council, you can make an appointment with any of the ophthalmic opticians on the list below to arrange your eye check. If you live in North Yorkshire, the mobile retinal screening service will visit your general practice and your yearly screening check will be organised by your family doctor.
