Wakefield District and North Kirklees Diabetes Network
What is a diabetes network?
The National Service Framework (NSF) for Diabetes : Standards, published by the Department of Health in December 2001, set out a vision for diabetes service improvement over the next 10 years. This was following by a delivery strategy in January 2003 which proposed the setting up of a local diabetes network.
Such a network should be formed around a “natural health community”. This means an area which is served by an acute hospital (in our area the Mid Yorkshire Hospitals NHS Trust which includes Dewsbury and District Hospital, Pinderfields General Hospital and Pontefract General Infirmary) and the Primary Care Trusts which “feed” into them (for our area Eastern Wakefield Primary Care Trust, North Kirklees Primary Care Trust and Wakefield West Primary Care Trust).
In April 2005, based on these guidelines, the Wakefield District and North Kirklees Diabetes Network was formed, which covers a population of 500,000 and an estimated population of 20,000 of people with diabetes.
The Diabetes National Service Framework is based on people with diabetes taking more control of their care. Not only at an individual level – the relationship between the person with diabetes and the health care team – but also by ensuring that people with diabetes (and parents of children and young people with diabetes) have a strong voice in determining local services and priorities.
The aim of the network therefore is to lead the implementation of the Diabetes National Service Framework, working closely with those with this condition and their carers in order to improve services.
Where do you receive your care?
The majority of patients are cared for in primary care by their GP and Practice Nurses. For those who need specialist hospital diabetes services, these are based at Pinderfields General Hospital, Pontefract General Infirmary and Dewsbury and District Hospital.

Edna Coates Diabetes centre is a purpose built centre in the grounds of Pinderfields General Hospital.
We see approximately 200 patients each week for Diabetes and endocrinology out patient appointments, Podiatry, dietetic and specialist nursing clinics.

Pontefract Diabetes Centre is situated in the grounds of Pontefract General Infirmary. Approximately 1500 patients with diabetes attend regularly seeing Doctors, Specialist Nurses, Podiatrist and Dietitian. A drop-in facility is offered too.
The Endocrine Clinic is held in Main Out-Patients and the Paediatric Diabetes Clinic is held in Paediatric Out- Patients.
